New Patient Forms

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time you will save significant time at the time of your visit. If you have any questions when filling out these forms, please do the best you can, our staff will assist you with your questions on the day of your visit.

Please printout and complete the appropriate forms:

• Vein History
• Medical History
• Registration Form

Appointments

• Scheduled Monday through Thursday
• Bring your current insurance cards
• Copays are collected at the time of service

Rescheduling or Cancellations:

• 60-90 minute appointments - Require 3 week notice
• All other appointments – Require 48 hours or by 4:30 pm on the Thursday prior to the appointment date
• A fee may be assessed if proper notice is not given

Insurance Information

We accept health plans offered by most major insurance carrier. No referral is necessary, unless required by your insurance.
